Westchester Jewish Community Services Counselor - Palliative Care in White Plains, New York

Overview:

Westchester Jewish Community Services (WJCS) is seeking a Part-Time or Full-Time Counselor Counselor to provide support services including care management to populations including individuals and families facing life limiting and chronic illness, geriatric clients, individuals with developmental disabilities, caregivers and referrals from synagogues in the Partners in Caring program.

The compensation range for this position is $30.00-$35.00/hour.

Responsibilities:

As a valued member of our team, your responsibilities will include, but are not limited to:

  • Provide intake and information-gathering sessions with referred clients

  • Provide care management as needed

  • Conduct individual and group sessions as needed.

  • Offer support to groups including but not limited seniors, people with developmental disabilities, people with chronic illnesses, and caregivers.

  • Referrals to other needed services such as home health care or clinical services

  • Other duties as required.

    Requirements:

    We are seeking candidates who have the following qualifications:

  • Masters Degree in Social Work or related degree from an accredited school, License preferred.

  • In-depth knowledge of aging, family issues, illness, and end of life/bereavement issues.

  • Strong knowledge of community resources.

  • Ability to work with adults and families.

  • Strong organizational and documentation skills.

  • Ability to work as part of a multi-disciplinary team with other professionals and social workers.
