Rate of Pay: $24.68/Hour

· Under the supervision of the Community Health Director, works in collaboration with team members to provide professional nutrition services to promote and maintain the health of individuals, families, and the community. Performs nutrition screening and assessment, education, interventions in home, clinic, and community settings. Functions independently and utilizes professional standards of practice and a competent level of dietetic judgement. Demonstrates a high level of interpersonal, verbal, and written communication skills, organizational skills and the ability to problem solve in a variety of situations, and to work independently and as part of a cohesive team. Travel Required.

· Conducts nutritional screening and assessments with clients and community members and works with them to identify goals.

· Communicates and collaborates with colleagues and other members of the health care team and community partners as appropriate.

· Develops strategies and nutrition interventions to create care plans to address nutritional problems and deficiencies.

· Maintains knowledge of current recommendations and professional standards of practice.

· Provides information and resources on nutritional topics to staff, clients, and the community.

· Provides individual counseling for pregnant and lactating women, infants and children, and high-risk clients.

· Ensures that documentation is complete, accurate, timely, and in compliance with WIC program and agency policies and protocols.

· Fidelity to State accreditation minimum program requirements.

· Maintains patient records and patient information per protocol and adhering to HIPAA and confidentiality policies.

· Other duties as assigned.

· Possession of a bachelor's degree, plus additional advanced training of one to two years, licensure as an RD and national certification. It is required for the employee to possess a current Registered Dietician Certification upon hire.

· Extensive experience in providing services to children ages 0-5 (beginning at birth) and their parents.

· Experience in working with culturally diverse communities and families; and the ability to be culturally sensitive and appropriate.

· 1-3 years of experience preferred.

· Knowledge of community resources/services necessary.

· Successful completion of CPR training and certification is a condition of continued employment.

· Initial/annual training pertaining to MI-OSHA's Blood Borne Pathogen Standard is a condition of continued employment.

· Availability of personal transportation and driver's license.

· Computer proficient.
