    Position Summary:

    The Community Health Worker (CHW) Supervisor will be responsible for supervising and supporting the day-to-day functions of the CHW team. In addition to managing a small panel of clients, the Supervisor will serve as the secondary CHW across the various CHW initiatives. Other key responsibilities include staff development, ensuring adherence to standards of professional practice, ensuring the provision of client-centered care and services, and engaging in quality improvement and evaluation activities. The CHW Supervisor will also assist the Manager in planning, design and development, and will also assist with the organization of community health promotion activities and events, as well as outreach publicity and recruitment.

    EDUCATIONAL REQUIREMENTS Bachelor's Degree in Social Work, Healthcare, or related field or Experience in Lieu of Education - Two years of relevant Community Health Worker, Community Outreach, or health education CERTIFICATION & LICENSURE REQUIREMENTS Driver's License Illinois upon hire and Proof of Auto Insurance upon hire and Certified Medical Assistant (CMA) National Health Career Association (NHA), National Association for Health Professionals (NAHP), American Medical Technologists (AMT), American Association of Medical Assistants (AAMA), National Center for Competency Testing (NCCT), American Allied Health (AAH) or the American Registry of Medical Assistants (ARMA) within 1 year or Registered Medical Assistant (RMA) American Medical Technologies (AMT) or the American Registry of Medical Assistants (ARMA). In addition, this certification requires: Successful completion of a Registered/Certified Medical Assistant (RMA/CMA) program or equivalent. within 1 year. EXPERIENCE REQUIREMENTS Familiarity and experience working with underrepresented populations. AndAbility to provide leadership, make difficult decisions, and be consistent and calm during challenging situations. AndMinimum of 2 years- experience as a Community Health Worker or a similar/related position AndMinimum of 2 years- experience as a supervisor to front line staff AndExperience conducting home visits, especially related to any chronic conditions, preferred. ADDITIONAL REQUIREMENTS

  • Community Health Worker certification through an approved program

  • Community Health Worker leadership workshop through an approved program

    Essential Functions:

  • Promotes growth of CHW team members in areas of patient-centered care, education, and cross-organizational care by providing continual coaching support and guidance in promoting adherence to the CHW Care Model and other evidence-based practice.

  • Serves as the subject matter expert on Community Health Worker (CHW) workflow and care model, providing leadership, guidance, and best practice techniques to CHW team.

  • Maintains a limited client caseload. Conducts social determinants of health screening to identify needs and develops plan to address health equity needs while assisting the client with problem-solving barriers to health through identifying, locating, connecting to, and navigating needed community and medical system services. This may also include accompanying clients to appointments and assisting with completion of forms to access needed services.

  • Assists with recruitment, hiring, development, evaluation, and retention activities including overseeing the orientation, continuing education, and competencies for new and current Community Health Workers.

  • Provides one to one reflective supervision for a team of up to 8 Community Health Workers. Will participate in case conferences, team meetings, and field supervision of team members.

  • Serves as a lead community representative by attending community coalition and workgroup meetings, making presentation on behalf of the programs, and fostering collaborative relationships with community partners, community resources, health plans, and providers.

  • Builds individual/client, community, and team capacity. Educates clients on how to obtain care and self-manage their conditions. Attends interdisciplinary care team discussions. Serves as a spokesperson and advocate for clients when they are unable to speak for themselves.

  • Ensures adherence to established standards of care and scopes of practice while assessing and developing tactics to address knowledge and skill gaps of Community Health Worker staff. Provides professional development opportunities to remediate gaps.

  • Develops and implements external and internal systems to assure timely receipt and disposition of referrals to and from the service.

  • Engages in program planning: Assist in developing program models and curriculum around new CHW programs. In this function, the CHW Supervisor would serve as the CHW and/or primary point person.

  • Plans and coordinates outreach, supervise individual and family assessments, assess training needs and provide training for CHWs as needed.

  • Facilitates case conferencing with CHW team to discuss client cases and provide mutual support.

  • Is available on an -on-call' basis to consult with CHWs and provide intervention support and an action plan for individuals or families in need

  • Assures quality service delivery and oversee data collection and reporting.

  • Supports CHW Team to administer assessments and assist patients/families to identify needs, provide basic health information, and make appropriate referrals to community services through home visits and community outreach.

  • Supervises a culture of caring, trust, as well as support and growth with clients served, community members, and other team members.

  • Works closely with the Manager on day-to-day operations.

  • Assign incoming patient referrals and monitor volume of referrals assigned to CHW staff and ensures they have the needed tools and resources to achieve goals and ensure compliance.

  • Provide intensive and ongoing supervision to the CHW team as they engage a targeted patient/community member population around health issue areas. Some of the supervisory functions, include but are not limited to: ongoing evaluation of CHWs skills; developing of tools to document CHW’s progress; providing regular feedback communications; ensuring CHW’s are timely in attendance and job functions; monitoring and assessing progress on program.
