    $37,264.00 - $41,218.00 Annually $1,433.00 - $1,585.00 Bi-weekly

    Position Summary

    This is a professional position, responsible for assisting individuals with the application process for agency services. This position consults with individuals, families, legal representatives, and other stakeholders to help them gain understanding of the APD eligibility process and programs and services offered by the Agency.

    The Work You Will Do

  • Assists individuals with the intake and application process by telephone, email, mail, online, and in person.

  • Process all assigned APD application packets and ensure all are mailed out within twenty-four (24) hours of completing the screening form and document the application process within system tracker.

  • Assist individuals in gathering documentation required for the application and eligibility determination process. Arrange for comprehensive assessments when needed to verify eligibility.

  • Issue notices regarding applicant eligibility, ineligibility, application status, or documents needed during the eligibility determination process.

  • Initiate and complete the short version support plan once eligibility approval has been determined. The intake coordinator will have 45 days after the application has been signed by the individual and/or their legal guardian to complete the support plan and the required initial Home Community Based Eligibility Worksheet.

  • Input, update and accurately maintain all referral and applicant information and forms in the iConnect and or other electronic systems utilized by the APD Agency within (5) five calendar days.

  • Assist individuals in the high-risk category with accessing appropriate services and referrals. Conduct regular reviews of case files for individuals in the high-risk category to ensure that an eligibility determination can be made prior to the individual reaching age 5.

  • Engages in Emergency Preparation and Response activities as directed.

  • Perform other related duties or assignments.

    Minimum Qualifications

  • High school Diploma or its equivalent.

  • Must have valid Driver’s License or other efficient means of transportation to travel for work purpose.

  • Must have knowledge and at least two years professional work experience with Developmental Disabilities or Mental Illness, or the principles and practices of one of the allied health field principles or one of the rehabilitative sciences.

  • Must have case management experience

  • Must have customer service experience.

  • Preference will be given to candidates who are fully bilingual (English & Spanish speaker).

    Demonstrate Skills in:

  • Working with Developmental Disabilities or Mental Illness, or the principles and practices of one of the allied health field principles or one of the rehabilitative sciences.

  • Interviewing applicants for public benefits assistance and services.

  • Skills in using Microsoft Office programs, applications and Web based programs.

    Direct Deposit Program

    As a condition of employment, a person appointed to a position in the State Personnel System is required to participate in the Direct Deposit Program. Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. This will be accomplished by Electronic Funds Transfer. Banks, savings and loan associations, and credit unions are eligible to accept such deposits. Retirement funds are also required to be in the Direct Deposit Program.

    Background Screening Requirement

    It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.

    No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.

Level 2 background screening shall include, but not be limited to, fingerprinting for Statewide criminal and juvenile records checks through the Florida Department of Law Enforcement, and Federal criminal records checks through the Federal Bureau of Investigation and may include local criminal records checks through local law enforcement agencies.

Pursuant to S. 110.1127(1), Florida Statutes, this position requires a background investigation, including fingerprinting.
