Summary:

Responsible for concurrent auditing of medical record documentation to ensure specificity of diagnoses and procedures to adequately reflect care provided, use of resources and diagnoses treated. Facilitate modifications to clinical documentation through extensive concurrent interaction with physicians and clinical staff to support that appropriate reimbursement and clinical severity is captured. Proactively coach physicians and clinical staff in areas where further specificity is needed in documentation. Supports timely, accurate, and complete documentation of clinical information. Educates the physicians and clinical staff on an ongoing basis.

Responsibilities:

Audits the Electronic Health Record and contacts physician as needed for further documentation and specificity.

Works collaboratively with physicians and follows up with recommendations to ensure that documentation is complete at discharge.

Monitors length of stay per working DRG and reevaluates account in a timely manner.

Works with the coding specialists to identify areas for documentation improvement.

Educates the physician on proper documentation requirements.

Queries the physician and clinical staff as needed to facilitate modifications to clinical documentation.

Assists the coding specialists on post-discharge documentation questions.

Reviews “no mcc and no cc” accounts within 24 hours of coding to see if clinical documentation leads to a physician query.

Experience as a Clinical Documentation Auditor preferred.

RN, BSN, LPN, RHIT, RHIA, CDIP or CCS
