Summary Advanced Registered Nurse Practitioner - Infectious Disease Section (HIV) Responsibilities The Infectious Diseases (ID) HIV ARNP is an integral member of the CVHCS ID Patient-Aligned Care Team (PACT), a team of providers that provides comprehensive primary care services in addition to HIV specialty care for approximately 300 veterans living with HIV. The ID PACT team also provides HIV prevention services, including comprehensive STI screening and prevention, to high risk HIV negative veterans. The ID HIV ARNP functions as the ID PACT clinic coordinator and primary point of contact for veterans receiving care in our ID PACT. In addition to the ID HIV ARNP, the ID PACT providers include three ID physicians, two ID Pharmacists, an ID RN, and 2 ID Fellows. ID PACT care is provided in the CVHCS medical specialties outpatient clinic and virtually, utilizing telehealth services when appropriate. Duties/tasks include, but are not be limited to: -Coordinates ID PACT clinic activities and schedules to help optimize clinic flow and ensure easy access for our veterans. -Coordinates and provides outpatient primary and HIV specialty care as well as follow up for inpatient Infectious Diseases patients, assuring that patient testing, history and physical examinations, discharge planning, and education are completed in a timely manner. This includes ordering laboratory, radiologic, and other diagnostic tests and providing initial interpretations and formulating differential diagnoses and developing treatment plans. -Reviews all laboratory, radiology reports, diagnostic testing, consultative reports for patients seen in Infectious Disease clinics and reports these results to patients as required by VA directives. -Screens patients for disease prevention, identifies health risks, and provides care to reduce health risks and preventable diseases, as pertains to primary care and HIV specialty care. -Serves as an agent of the Infectious Disease physicians regarding prescriptive authority, in conjunction with the Infectious Diseases Fellows. -Assesses patient's learning needs and adherence to care, develops and implements plans to meet patient needs, and instructs patients and caregivers in self-care and treatment. -Participates in the work of the Infectious Diseases PACT clinics, and the Infectious Diseases Consult clinic and Infectious Diseases Ad hoc clinics as needed. Manages scheduling and arranges appointments for select patients in these clinics. -Triages patient/family/agency telephone calls regarding patient care issues. Maintains contact with professional, supervisory, and regulatory agencies regarding patient care reporting and administrative obligations. -Continuously reviews and revises patient care based on clinical findings and assessment while following approved clinical protocols and privileges. In addition, develops protocols or clinical pathways to standardize care for Infectious Diseases patients to expedite and optimize patient care delivery. -Reviews all positive HIV tests obtained within CVHCS and ensures newly diagnosed HIV positive patients are properly notified and linked to care with a trained HIV provider in a timely manner. Notifies public health departments of newly diagnosed HIV patients. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Master's or Doctoral Degree from a program accredited by the Accreditation Commission for Education in Nursing or the Commission (ACEN-Formerly NCLAC) or the Collegiate Nursing Education (CCNE). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Nurse Practitioners must be licensed or otherwise recognized as a nurse practitioner in a State and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected. Preferred Experience: Previous internal medicine and/or primary care experience required; HIV specialty care experience strongly preferred. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level III - Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. This position requires potentially long periods of continued walking, standing, and sitting, lifting, carrying, pushing, reaching above shoulder, use of fingers, both hands, both legs and repeated bending while wearing protective lead. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders; Emotional and Mental Stability: The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. Work Environment: Work is performed primarily in a patient care setting; adequately lighted, temperature controlled, and ventilated. Office temperatures fluctuate periodically; involves everyday risks or discomforts which require normal safety precautions typical of such places as clinics, offices, meeting and training rooms or libraries; exposure to noise and dust on occasion. The work area is adequately lighted, heated, and ventilated. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures.
