Shift: Sunday through Thursday; or Tuesday through Saturday

For nearly 60 years, Calvary Healing Center has provided a full continuum of care, specializing in addiction treatment for adults across the Phoenix area and Nationwide. Calvary’s proven treatment programs address the practical, physical, and spiritual aspects of addiction for lasting recovery. Our flagship location in Phoenix offers onsite detoxification and residential treatment programs, a partial hospitalization program (PHP), and an intensive outpatient program (IOP). We also have a satellite clinic offering Intensive Outpatient Programs in the West Valley region. Through our programs we offer many other holistic healing and treatment options complementing our services ranging from yoga, art workshops, and music.

    Case Manager Position Summary:

    The Case Manager works under the guidance of the Clinical Director while mentoring case management staff. The Case Manager collaborates with other healthcare team members to coordinate and facilitate the effective delivery of quality servics to Calvary patients. The Case Manager is knowledgeable about and responsible for treatment plans, utilization review, discharge planning, psychoeducation groups, and case management.

    Qualifications

    Case Manager Job Requirements:

  • Bachelors degree required, Master’s degree, preferred.

  • Minimum of 2 years of experience in case management related work.

  • Completion of BLS & First Aid (FA) training within 30 days of employment.

  • Completion of Crisis Prevention and Intervention (CPI) training within 30 days of employment

  • Must have a valid driver's license and agree to a Motor Vehicle Driving Record check as part of the background check. And that record must be in good standing.
