Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

Currently, we are seeking a On Call Resident Support Specialist to work in our Homeless Services Division. This position has responsibilities of facilitate participant growth and development toward self-sufficiency in daily living skills, socialization and community living and facilitating participants' stabilization in the community. Some other responsibilities include the following: Facilitating and reinforce skill development in the following areas: social supports, food management, self-care preservation/personal hygiene, relapse prevention, medication management, stress management techniques, money management, utilization of public transportation and participation in meaningful and planned community activities. Completing documentation, including progress notes and service plans, as required by regulations. Providing resolution and follow-up to crisis or problems, in conjunction with managerial input. Serving as an advocate for participants to assume a proactive role in their recovery. Maintaining and monitor medications consistent with individuals' self- medication plans, in compliance with State and Agency medication administration regulations. Establishing an on-going dialogue with other members of the treatment team, including program staff, vocational/educational instructors, consultants, members of the medical profession and family members to provide continuous supports. Performing other duties and responsibilities as they relate to the overall enhancement of program service delivery.

Job Qualifications:

Associates degree and 2 years experience working in the mental health field or High school diploma and 3 years experience in the mental health field; or any combination of education, experience and training that would provide the level of knowledge, skill and ability required.

Demonstrated crisis intervention/prevention and de-escalation skills required.

Proficient communication skills, both oral and written.

Ability to work independently and as part of a team.

Some knowledge of the behavioral health system and supports.

Scope:

Frequent talking and listening.

Requires the ability to lift carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with residents.

Possess manual dexterity and fine motor skills.

May require crisis intervention in emergency situations.

May be required to report to work during emergencies including inclement weather.
