Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

Currently, we are seeking a full time Psychiatric Rehabilitation Worker to work in our Behavioral Health Services Division. This position has the responsibilities of providing Psychiatric Rehabilitation Services (PSR) facility based and/or in the community as assigned, facilitating groups/units to enhance an individual's knowledge to develop, enhance and retain skills and competencies in living, learning, working and socializing. They may also help individuals receive the level of and/or have access to the services/supports they choose, so that an individual can live in the environment of their choice and monitor and track progress of individual's in assigned caseload; participate in the development of individualized rehabilitation plans. Some other responsibilities include the following:

Provides and facilitates individual's skill development and support which includes daily living, socialization, educational/vocational supports and opportunities, and leisure/recreation activities planning including community integration and community connections. Maintain and monitor an assigned caseload of individual's. Participate in the analysis and identification of service delivery gaps, changes and problems under the guidance of the supervisor. Consistently meet productivity expectations as set by administration, department and funding sources. Participate in the resolution of crisis situations; assist with identifying individuals and resources that can provide direct support and/or immediate intervention during a crisis situation. Complete and assist with required documentation. Develop, review, revise forms, procedures and standards of care according to accepted professional practice. Establish and maintain liaisons with other service providers to promote program coordination and enhance individuals services. Participate in relevant training to enhance professional growth, and development with an emphasis on psychiatric rehabilitation services and evidence based practices. Serve as advocate and encourage individual's to assume a proactive role in their rehabilitative process, including mastery of skills.

Job Qualifications:

Bachelors degree, or Associates degree and 1 year work experience in mental health direct service, or CPS certification and 1 year paid or volunteer experience, or a High School diploma/GED and 2 years work experience in Human Services, which must include 1 year of mental health direct service.

Must have excellent oral and written communication skills, as well as be detail-oriented and able to complete a high volume of documentation in a fast-paced environment.

Strong computer skills are required, as well as Internet research abilities.

Ability to work independently and as part of a team.

Knowledge of mental health system and supports.

Must possess a valid driver's license with an acceptable driving record.

Scope:

Frequent alking and listening.

Requires lifting and/or carrying and the ability to fold, kneel, stack, stoop and bend which may include direct client intervention.

Must be available for local travel.

May require crisis intervention for emergency situations.

Participation in on call coverage as applicable.
