Bon Secours Mercy Health Registered Nurse (RN) Rehab Coordinator- Acute Rehab- St. Charles Hospital in Oregon, Ohio

Rehab clinical Assessment Coordinator, in conjunction with the Acute Rehab Unit Medical Director, coordinates the referral development and pre-admission patient assessment process and provides clinical assessments for potential patients.

Coordinates the admission process to the Acute Rehab Unit and communicates pertinent information to all involved parties (patient, family, physician, payor source, case manager, interdisciplinary team).

Provides on and off site clinical assessments for potential patients to the acute rehab unit from the metro area. Documents all assessment data.

Establishes and maintains effective working relationships with patients, families, co-workers, physicians, payor sources, hospital administration, community members and referral sources.

Conducts all insurance and payment arrangements during the admission process and communicates and documents in appropriate areas.

Follows up on all patients who were not admitted to the acute rehab unit to determine reason for non-admission. Assists with alternative placement recommendations if not appropriate for the acute rehab unit.

Consults with Clinical Nurse Manager and Medical Director regarding pre-screen assessments and physician consultations to determine appropriate placement of potential patients.

Works with the ARU Leadership Team to ensure the program meets all local, state, TJC and CARF regulatory standards.

Identifies areas needing improvement and utilizes the hospital performance improvement process. Actively participates in comprehensive program evaluation.

Serves as educational resource to patients, families, interdisciplinary team members, payor sources, physicians, case managers, community members and other parties as appropriate regarding rehabilitation issues.

Interacts with interdisciplinary treatment team to ensure incorporation of appropriate patient and family goals within the treatment plan.

Is cross trained to cover for the PPS Coordinator.

Minimum Qualifications

Minimum Years and Type of Experience:

5 years’ experience as an RN with experience in an acute rehab

Other Knowledge, Skills and Abilities Required:

Analytical abilities to assess patient’s nursing, social and discharge planning needs. Excellent communication and collaboration skills to work effectively with patient, family, physician, nurses and other health care team members. Computer skills required.

Other Knowledge, Skills and Abilities Preferred:

FIM, knowledge of Community Resources.
