Mille Lacs Health System Activities Coordinator LTC (1.0 FTE) in Onamia, Minnesota

Summary

The Activity Coordinator is responsible for the daily operations of the Activities Department in Long Term Care (LTC) and Hospital Swing Bed patients. This position is part of the interdisciplinary team that participates in planning and implementing resident and patient care.

Job Duties

  • Responsible for individualized assessment and care planning for residents that meets the residents' needs, abilities and interests, providing individual assessments. Ensures that care plans (activities area) are delegated to appropriate staff and/or volunteers and that the care plan is followed and goals are met or revised as necessary. Participates in resident care conferences.*

  • Plans activities for the LTC residents and Swing Bed patients. This planning includes daily activities and programming, major department events, entertainment, and spiritual care. Develops program plans for supplies, communication (residents, families, staff) and other items needed to deliver successful activities and events.*

  • Keeps abreast of current activities programming best practices and brings suggestions and planning to the DON.*

  • Responsible for preparing annual operating and capital budgets and other financial recommendations to the DON.*

  • Is an active participant in the LTC Interdisciplinary Team. Ensures open, constructive communication with other areas such as Dietary, Nursing, Social Services and others involved with care of residents and patients.*

  • Delivers direct patient care (activities) including Dining Room supervision, feeding assistance, and activities to ensure patient care needs are met.*

  • Plans for regulatory compliance and participates in the MDH survey process.*

  • Is responsible for the correction of deficiencies pertaining to the Activities department.*

  • Responsible for annual Process Improvements for the Activities Department and submits reports to the Quality Department as assigned.*

  • In charge of the LTC Newsletter in cooperation with Marketing and distributes to families.*

    Required Education and Experience

  • Recreational Therapist, Certified Activity Professional, NAACP Certified, COTA, or an equivalent technical degree.

    Additional Eligibility Qualifications

  • Demonstrate the knowledge and skills necessary to provide care to the resident population serviced in the Long Term Care and Hospital Swing Bed patients.

  • Demonstrate knowledge of the principles of growth, development and aging, from adolescence through the end of life needs.

  • Assess and interpret data relating to the resident's status in order to identify the resident's age-specific needs.

  • Good interpersonal and communication skills (verbal, nonverbal and listening).

  • Computer skills including Word, Excel, and Outlook.

    Position Type/Expected Hours of Work

    Profession: This position is scheduled Monday through Sunday with eight (8) hour day, and evening shifts, rotating weekends and holidays.

    Supervisory Responsibility

    Supervises activity staff under the direction of the Director Of Nursing
