Continuum of Care Wellness Support Staff in New Haven, Connecticut

Department:Mental Health Services

Scope of Service: Provides a comprehensive array of community-based rehabilitation focused on skill building with a goal of maximizing independence. Provides services in a team approach which include but not limited to: intensive, rehabilitative community support, crisis intervention, group and individual psycho-education, and skill building for activities of daily living. Ensures that services and interventions are highly individualized and tailored to the needs and preferences of the individual. Models and delivers health and wellness care and coaching for Agency clients. Strongly understands the importance of healthy nutrition and daily activity, as well as how it supports individuals with mental illness and addiction disorders towards their recovery. Coordinates and facilitates health services, groups, classes, and presentations in order to promote health and wellness among this population. Provides hands-on, intensive skill-building interventions and supports to clients to enrich and achieve an overall level of independence and well-being. Performs other duties as assigned. QUALIFICATIONS:

  • Bachelors degree preferred with a minimum of three (3) years experience working in related behavioral health field with a concentration in providing health coaching. Associates degree or lived experience considered with a minimum of five (5) years relevant experience.
  • Certification in DARC, health, wellness, and/or fitness preferred.
  • Demonstrates an understanding of the recovery process.
  • Prior experience working with people with mental illness and substance use disorders is required.
  • Valid Connecticut Drivers license is required. Must have reliable transportation and a willingness to transport clients in personal insured vehicles.
  • First Aid/CPR certification or willingness to obtain certification within 90 days of hire. Employee is responsible to maintain ongoing certification.
  • Annual physical, PPD, and other health requirements as regulated by Continuum and the Department of Public Health.
  • Proficient computer skills required, including Microsoft Office and electronic record keeping.

  • PT 16 hours Fri/Sat 3PM-11PM
  • PT 16 hours Fri/Sat 11PM-7AM

Luna's Path - New Haven, CT:

  • PT 16 Hours Sun/Mon 4PM-12AM
  • PT 16 hours Sat/Sun 11PM-7AM

Full time/ Part time $20.03 Per diem all shifts available: $18.83
