JOB SUMMARY

· Responsible to Rehabilitative Services Director In/Outpatient.

· Provides physical therapy evaluation and treatment to patients upon appropriate medical referral and/or as medically necessary.

· Performs related duties of documentation, communication with physicians and other ancillary services and patient charges.

· Participates in department meetings, in services, audits, Performance Improvement, and discharge planning when appropriate.

· Processes confidential information and adheres to Patient Bill of Rights.

· Provides holiday and weekend coverage as needed.

· Will provide coverage in other physical therapy departments as needed and determined by Director of Rehabilitation Services and immediate supervisor

JOB REQUIREMENTS

Education:

· Graduate of an accredited program in Physical Therapy with at least a Bachelor of Science Degree.

· Licensed in the State of New Hampshire prior to beginning employment.

Other Skills Required:

· Completes all mandatory competency testing.
