Registered Nurse (RN) Home Care Coordinator

Responsibilities Will Include:

Has the responsibility for providing direct and indirect nursing care to assigned participants. Works under the direct and indirect supervision of the Center Manager or designee but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which he/she is responsible.

  • Assesses the home care needs and home safety of a frail elderly population. Contributes to development of specific plans of care. Initial and periodic assessments to be conducted minimally every 6 months, complying with PACE regulations. Assist in determining Level of Care Determination for potential and current participants.
  • Aid in organizing the provision of care around-the-clock. Assist in the coordination and implementation of homecare services, including nursing, PT, OT, ST, and personal care services, to guarantee that the high-quality services fulfill the needs of the residents.
  • Must interact efficiently and respectfully with other members/disciplines of the interdisciplinary team.
  • May need to assist in conducting competencies of PCAs upon orientation
  • Demonstrates active participation in QI processes
  • Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning.
  • Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members

Qualifications:

Education: Registered Nurse (RN) with a current Michigan License

Experience: At least one year experience as an RN, preferably in either home or community health. Supervisory experience desired.

Licensure, Registry or Certification Required: RN currently licensed in Michigan; presents credentials which demonstrate ability to meet performance criteria for RN; CPR certification.

Special Training: Must have at least one year of experience in working with the elderly population. Only act within the scope of their authority to practice. Before working independently, one must first satisfy a set of standards-based competencies created by PACE Central Michigan and endorsed by CMS.
