POSITION SUMMARY:

The Nursing Assistant is responsible for providing direct nursing care under the direction and supervision of licensed nursing personnel and in accordance with Health Care Center policies and procedures and the employee manual.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  1. Resident Care Responsibilities
    • Perform, or assist resident in bathing, dressing, grooming, oral care, personal hygiene, toilet functions, and nail, foot, and skin care. Grooming refers to the removal of facial hair and hair care, which includes daily combing/brushing, setting and styling.
    • Assist with admission, transfer and/or discharge of residents as directed by nurse in charge.
    • Assist with meal service, including passing and serving of trays, feeding, and timely pick up. Offer fluids and nourishment to residents throughout the shift when appropriate. Provide after-meal care as needed.
    • Perform specialized care for residents, including but not limited to:
      1. Eye and ear care.
      2. Catheter and colostomy care.
      3. Bladder and bowel functions.
      4. Vaginal irrigation/douches.
      5. Changing dressings, bandages, binders, etc. as instructed.
      6. Providing physical, respiratory and reality orientation therapy as instructed.
      7. Assisting with application of compresses as instructed.
      8. Collecting specimens for medical tests as instructed.
      9. Proper usage of oxygen equipment.
    • Assist with care of dying residents and post-mortem care.
    • Assist with lifting, turning, moving, positioning, and transporting residents. Follow proper body mechanics and use appropriate lift equipment as needed.
    • Assist in the care of bedfast residents as requested.
    • Give backrubs as instructed.
    • Give special attention to:
      1. Keeping incontinent residents clean and dry.
      2. Positioning bedfast residents for comfort.
      3. Maintaining comfort level of residents and surroundings.
      4. Keeping resident's call-light within reach.
      5. Observing instructions of nurse in charge regarding bed rails.

Observation and Reporting

  • Report all accidents/incidents on shift to nurse in charge.
  • Report all changes in a resident's condition to nurse in charge.
  • Report all complaints/grievances of residents to nurse in charge.
  • Maintain intake and output records on residents as instructed.
  • Take residents' vital signs and record as instructed. Weigh and measure residents and record as instructed.
  • Report presence of pressure areas and skin breakdowns to nurse in charge.
  • Observe disoriented and comatose residents and report data as instructed.
  • Report to the nurse in charge the finding of any medication in a resident's room.
  • Record resident's food/fluid intake as instructed and report any changes in a resident's eating habits.
  • Notify nurse in charge of any resident missing from the facility.
  • Report defective equipment and/or hazardous conditions to nurse in charge. Inform nurse in charge of any needed supplies/equipment.

Rehabilitation

  • Encourage residents to do as much as possible for themselves.
  • Assist in carrying out rehabilitative procedures such as range of motion exercises, walking, turning and positioning, bowel and bladder training, use of assistive devices, feeding and re-learning ADL skills.
  • Assist residents in walking with or without self-help devices as instructed.
  • Perform restorative and rehabilitative procedures as instructed.

Change and make beds.

Keep residents' rooms and belongings neat, clean, and orderly.

Clean and disinfect equipment as instructed.

Use supplies appropriately and replenish them in residents' rooms.
