  • This position is located at our Woodmore clinic in Mitchellville, MD

Description:

  • Reporting to the Manager of Dialysis

  • Social Worker will be responsible for assessing, counseling, referral and supportive services to patients and their families in a manner consistent with professional standards and institutional mission and policies.

Responsibilities:

  • Collaborates with physicians, medical professionals and other staff to ensure the quality of care given.

  • Provides care for the adult or geriatric patient (as defined in the unit scope of care).

  • Utilizes knowledge of the principles of growth and development appropriate to the patient’s age to ensure that psychosocial needs of the patient are met.

  • Involves appropriate parties (i.e. patient’s family or guardian, outside agencies) in the psychosocial care of the patient as needed.

What you will need:

  • Master’s degree in social work from an accredited university.

  • Current LCSW licensure by the State of Maryland or eligibility for licensure.

  • At least one year post Master’s Degree experience as a social worker in a health care environment, renal experience preferred.

  • Demonstrated ability to work with all age groups of patients, with an emphasis on adult and geriatric patients.
