Fairview Health Services Clinical Pharmacy Specialist in Minneapolis, Minnesota

Overview

M Health Fairview has a clinically focused pharmacy position open to provide pharmaceutical care to patients, provide consult services to physicians, support the research and educational objectives, and serve as informational resource to colleagues.

In this position you will serve as the clinical pharmacy specialist for pediatric antimicrobial stewardship and will perform the responsibilities below as relates to pediatric infectious diseases and antimicrobial stewardship.

This is a fulltime benefit eligible role!

Responsibilities Job Description

  • Serving as the clinical pharmacy specialist for pediatric antimicrobial stewardship you will perform the responsibilities below as it relates to pediatric infectious diseases and antimicrobial stewardship.

  • Provide pharmaceutical care to patients including patient assessments, identifying and documenting drug therapy problems, resolving drug problems and providing drug information.

  • Coordinate pharmacy services with pharmacists and technicians.

  • Participate in departmental initiatives, including formulary and cost-reduction initiatives.

  • Develop pharmacotherapy expertise for a select patient group; serve as a resource for the care of these patients.

  • Foster an environment of departmental teamwork with other pharmacists, technicians, residents, and students.

  • Develop and maintain interdepartmental relationships with physicians, nurses, and other healthcare professionals.

  • Serve as primary preceptor for the residency program and clerkship students from the College of Pharmacy.

  • Provide drug information for medical staff, nursing staff and patients.

  • Participate in performance improvement and medication error reduction initiatives.

  • Participate in team meetings.

Qualifications

Required Education

  • Bachelor’s degree in Pharmacy

  • PGY2 pharmacy residency training in either Infectious Disease or Pediatrics OR equivalent experiences in those specialty areas

Required Experience

  • 0-1 year

License/Certification/Registration

  • Active Minnesota Pharmacist License

Preferred Education

  • Pharm D

Preferred Experience

  • 1 to 3 years

License/Certification/Registration
