Health First Intake Referral Coordinator - Family Pharmacy in Melbourne, Florida

*Position SUMMARY: *

To be fully engaged in the coordination of Home Infusion referrals, to provide support for Senior Infusion Liaison, and to operate in Family Pharmacy workflow for Home Infusion orders. The Intake Referral Coordinator is responsible for providing the ongoing updates to referral sources and for the success of infusion coordination of care from referral source into the home setting on behalf of Health First Family Pharmacy Home Infusion (HFFPHI).

*Primary Accountabilities *

  1. Complete coordination of home infusion referrals through direct communication between referral source, patient, CarePath and/or HFFPHI team.
  2. Create, build, maintain and strengthen relationships with referral sources, patients, CarePath and HFFHPHI team.
  3. Ensure that the correct access, medication and supplies are included in orders for HFFPHI.
  4. Follow-up on all referrals until delivery has occurred.
  5. Participate in education and training for Home Infusion IV, counter and referral tasks.
  6. Maintain patient and departmental/hospital confidentiality.
  7. Attend pharmacy staff and CarePath meetings.
  8. Support Senior Home Infusion Liaison with any and all referral needs.
  9. Support Health First Family Pharmacy Hospice and Retail departments as time permits.

*MINIMUM QUALIFICATIONS *

Education:High school/GED. Certification:None. *Licensure: *

  • Registered Florida Pharmacy Tech License.
  • Clean State of Florida motor vehicle record and maintain.

*Work Experience: *2 years of pharmacy/healthcare experience.

  • Exceptional communication skills with internal and external parties.
  • Exceptional customer service skills.
  • Sound organizational skills to include time management and problem solving.
  • Proficient computer skills.

*PREFERRED QUALIFICATIONS *

Education: *No additional. *Licensure: *No additional. *Certification:National Certification for Pharmacy Technician (PTCB or ExCPT). *Work Experience: *

  • 5 years customer service experience.
  • 1 year of IV experience.

    *PHYSICAL REQUIREMENTS *

  • Majority of time involves sitting or standing; occasional walking, bending, stooping.

  • Long periods of computer time or at workstation.
  • Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
  • May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
  • Communicating with others to exchange information.
  • Visual acuity and hand-eye coordination to perform tasks.
  • Workspace may vary from open to confined; on site or remote.
  • May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
