A licensed MSW to provide care coordination, counseling, resources, and discharge planning for perinatal, NICU, and pediatric patients and families. Provides resources and choices to families, which effectively link them to the needed level of emotional, medical and spiritual care, including post-discharge resources. This individual is an active participant on the multi-disciplinary team and will receive referrals for perinatal, NICU, and pediatric patients from physicians, RN Care Managers, staff nurses and other members of the care team. Excellent communication skills and a collaborative approach patient centric care is required.

Develops discharge plan in direct consultation with patient, family, physician, and healthcare team. Deals with families exhibiting complex family dynamics that directly affects patient care and discharge.

Identifies high risk patients from a psychosocial/ financial perspective and provides information, support, counseling, care management, and referrals to appropriate resources. Serves as a liaison for complex transitions/discharges between the hospital and community agencies for the exchange of clinical and referral information. Contributes to training of social work professionals as part of the education program at LUHS.

Required: Masters Degree

Minimum Experience :

Required: 2-4 years of perinatal, NICU, and/or pediatric inpatient experience.

Licensure/Certifications:

Required : Licensed Social Worker State of Illinois

Preferred: Licensed Clinical Social Worker State of Illinois
