Lake Superior Life Care & Hospice Bereavement Coordinator/Social Worker in Marquette, Michigan

We are looking for an outstanding social worker /bereavement coordinator to join our team. This is currently a dual role position, but may be split into two positions with the right candidate(s). We are dedicated to making our patients final days, weeks and months as comfortable as possible.

About Us

Lake Superior Life Care & Hospice (LSLC&H) is a nonprofit, community-based organization serving Marquette County since 1979. As one of Michigan's oldest existing hospices, LSLC&H provides personal and professional care 24 hours a day 7 days a week. LSLC&H strives to comfort patients and their families during life limiting situations.

Hospice is palliative care for patients near the end of their life. Hospice care can be provided in a patients' home, hospice centers, hospitals, long-term care facilities, or wherever a patient resides. Palliative care focuses on improving a patient's quality of life by managing pain and other distressing symptoms of a serious life-limiting illness.

Position Summary

The Hospice Social Worker provides assessment and intervention to assist clients/families to improve social and economic difficulties interfering with health and wellness through use of casework process and principles, strategies, and community resources.

The Bereavement Coordinator role provides emotional care to terminally ill patients and their families as well as grief support to the family after the death of a patient. Responsible for developing bereavement care plans and programs and facilitating support groups.

Duties: What you will do:

  • Provide social work support to patients and families
  • Assist patients and families in obtaining needed services
  • Assist with preparing forms for an advance directive, do not resuscitate (DNR) order or a physician order for life-sustaining treatment (POLST).
  • Assist in filling out and filing insurance, Medicare and Medicaid paperwork
  • Find local counseling services and support groups for family members
  • Assist with intake of new patients & conduct initial psychosocial assessments of new patients
  • Complete social work and bereavement care plans
  • Facilitate grief support groups
  • Assist in the organization of the annual Memorial Service
  • Collaborate with the members of the hospice interdisciplinary team to identify and resolve problems
  • Participate as member of the hospice interdisciplinary team
  • Complete monthly phone calls to families
  • Assist families in understanding the grief process
  • Travel to patients wherever they reside: Home, nursing facility etc.

Requirements: What you need to have:

  • Bachelor of Social Work (BSW)
  • Valid social work license
  • Driver's license, insurance and vehicle
  • 1-2 years of healthcare experience (preferred)
  • Passion for working with those experiencing grief and loss
  • Utmost compassion and integrity

Job Types: Full-time, Part-time

Pay: From $24.00 per hour

Expected hours: 24 -- 32 per week
