Job Summary:

Responsible for the accurate and time sensitive completion of return to work, Family and Medical Leave Act (FMLA) and Disability related forms received by the Health Information Department.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Reviews and understands patient’s charted medical history, diagnosis and CPT codes to summarize abstracted answers on FMLA and disability forms.

  • Processes disability, FMLA and return to work forms from a variety of employers and insurance companies after the intake process has been initiated.

  • Answers and handles incoming phone calls from patients/customers, insurance companies, employers and clinical staff to provide information regarding workflow and form status. Handles difficult calls in a professional manner and provides solutions to resolve issues in a quick and efficient manner.

  • Validates the authorization to release records to a third party entity; ensures compliance with Health Insurance Portability and Accountability Act (HIPAA) regulations. Scans signed and completed forms and authorizations into the electronic health record.

  • Communicates with clinical staff and providers via electronic health record messages as needed to complete the forms in a timely manner.

  • Reviews finalized forms and ensures HIPAA compliance as well as accuracy and completeness of information provided on the form. Mails, faxes, emails or uses electronic health record messaging completed forms to patient or approved recipients.

  • Performs other duties as assigned.

EDUCATION

  • High school diploma or equivalent

EXPERIENCE

  • Two years' experience

PHYSICAL REQUIREMENTS

  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.

  • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.

  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.

  • Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.

  • Frequent keyboard use/data entry.

  • Occasional bending, stooping, kneeling, squatting, twisting and gripping.

  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.

  • Rare climbing.
