Job Summary: Provide nurse care management services to Providence and Residential guests and nursing care to persons experiencing homelessness in the community.

FLSA: Full Time (32 hours), Non-exempt

All Water Street Mission staff must be:

Essential Functions:

Culture and Values

• Share the Good News of Jesus Christ in words and actions with people we serve, communicating hope and dignity.

• Walk alongside fellow WSM staff in Christian community as we journey toward maturity.

• Encourage restoration in the lives of coworkers and guests, recognizing we are each restored by God to become restorers.

• Work in full coordination with other WSM staff at all times to identify, report and respond appropriately to any potential threats to the safety and wellbeing of guests.

Role Specific Responsibilities

• Assessment & Triage of medical issues for Providence Shelter guests.

• One on One consultation with guests during day shelter hours.

• Provide NCM services for guests with multiple, chronic medical issues, prioritizing Providence shelter guests.

• Make referrals to community health services and resources.

• Accurate and complete chart documentation and form completion in designated EMR.

• Teach and/or coordinate health related educational classes for guests and/or staff as appropriate.

• Attend off campus medical appointments with guests as needed.

• Collaborate with relevant program staff on guests' health related issues.

• Attend appropriate multidisciplinary team meetings.

• Coordination and facilitation of CPR and Narcan training for staff.

• Restocking of First Aid Kits and Spill Kits for all WSM departments.

• Other duties pertaining to the essential functions.

Education and Experience: An associate degree accepted a bachelor's degree preferred and a minimum of 2 years' experience in the nursing field. Unrestricted License to practice nursing in Pennsylvania. Experience working with patients that have a history of trauma.

Reports To: Director of Medical Services

Key Internal Relationships: Medical staff and volunteers, dental staff and volunteers, Access Center, Case management, Mission Directors, Key community medical providers
