The Mount Sinai Health System Health Education Coordinator I-MSH/QHC-12811-QHC-001 in Jamaica, New York

Under supervision of Project Director and/or Director of Service, responsible for community outreach, education, and direct patient services for the Smoking Cessation Program. Assists patients, in the inpatient and/or outpatient settings, in smoking cessation and relapse prevention, using therapeutic counseling techniques and standardized protocols. Communicates, as needed, with referring physicians and other hospital/clinic staff; conducts site-based Smoking cessation Intervention Program informational sessions and patient support groups, as needed; and performs administrative tasks related to patient care. Responsible for maintaining up to date information on current tobacco trends and medical policies related to tobacco dependence treatment. Collaborate with project partners to maintain records of current policy and practices of project’s participating medical groups. Develop relationships with health care provider organizations (departments, hospitals and clinics). Coordinating with these health care organizations and the medical staff to increase adherence to the smoking intervention best practices. This will include efforts to assure changes in the policy and practice of these health care organizations to maximize utilization of tobacco resources available to the Queens community. Organize conduct, initial and follow-up provider education programs. Coordinate follow-up with individual providers to ensure they have the necessary resources to reach the needs of their client population. Serves as active member of specified hospital committees, task forces, and professional organizations. Accountable for effectively coordinating any project related community education services. Utilizes independent and collaborative judgment in decision-making. Demonstrates leadership and problem solving skills. Incumbent should have skills in teaching techniques that encompass group dynamics. Incumbent should also have the ability to effectively communicate both orally and in writing including presentation skills. As needed, act as liaison with insurance companies to inform the medical community of tobacco related services available and policies in place by these companies.
