Description The RN MDS Case Manager is a member of the neighborhood team who works in partnership with direct care nursing staff to ensure that patient care is based on healthcare needs and patient preferences, and adheres to regulatory requirements. The Case Management Nurse's primary role is to complete a comprehensive assessment and care plan for each patient in her/his caseload. Required Qualifications: Washington Registered Nurse License upon hire. 2 years of Nursing experience in LTC, TCU or equivalent experience Preferred Qualifications: Bachelor's Degree in Nursing Why Join Providence?
