What You Need to Know Telehealth LVN Responsibilities Will communicate directly with field clinicians, physicians and/or clinical management if necessary to ensure timely care, including, but not limited to potential exacerbations in patient’s health monitoring. Will perform virtual reviews of medications at initial evaluation and ongoing per visit frequency to ensure compliance and ensure proper reconciliation. Will include following patient POC and adhering to documentation guidelines for timeliness and accuracy in the home Will be responsible for a case load and will be expected to follow daily cadence and meet goals established for daily, weekly and monthly completion of calls and documentation. Will include close communication with clinical management, educators and other staff on any identified needs in the field Will educate disease management zones, assess via triage and communicate with community care team to ensure patient remains in home reducing readmissions and ER visits Will have the end goal of maintaining patient engagement, ongoing communication with patients and families to ensure patient is stable and ensure the care the patient is receiving is in line with established goals of care Qualifications Telehealth LVN Qualifications LVN/ LPN licensure and willing to obtain licensure in each state as needed Possesses and maintains current CPR certification Experience with telehealth nursing preferred Minimum of one year experience in health care Experience working with the elderly/disabled ensuring that basic needs are met required One year experience in Medicare health settings preferred
