Corewell Health Medical Director, Population Health in Grand Rapids, Michigan

Job Summary

Accountable for developing and achieving population health tactics and targets for the ACO, CIN, and other risk-based programs. In collaboration with clinical service lines, oversees transformation of clinical practice to achieve optimal outcomes and cost reduction for value work within the continuum of care (inpatient, outpatient, prevention, and wellness).

Develops understanding of broad health landscape, determining key drivers and opportunities for innovative models targeting the Quadruple Aim. Uses qualitative and quantitative data to develop understanding of health needs and the attributes of the populations services to drive development of programs.

Lead quality initiatives. Monitor and assess key performance metrics, including patient experience, outcomes, cost and operational efficiency.

Essential Functions

  • In a collaborative relationship with leadership, the Medical Director assumes accountability for the delivery of quality services through consistent development, revision, implementation and interpretation of procedures, protocols, and standards of care effecting patients.

In a collaborative relationship with leadership, the Medical Director maintains accountability for the development of capital and operational budgets for the service. Monitors, evaluates, and advocates for appropriate utilization of resources within the service.

  • Enforces approved privileges for medical staff, interpreting boundaries and limits, as indicated, for quality patient care.

  • Participates in short and long range planning within the institution in order to provide effective and efficient delivery of care and improved continuity and coordination of services. Participates in program development activities, as appropriate. With the Director, develops and monitors the strategic plan for the service.

  • Takes a leadership role in the planning, implementation and evaluation of quality improvement / PI activities which support the overall mission of Medical Education, the mission / goals / vision of the health system, and quality patient care.

  • Acts as an advocate and provides ongoing communication with patients / families, staff and the multidisciplinary team regarding patient, staff, management, quality care issues and divisional / hospital goals. Works with the Chairman of the Department, and all other Medical Directors / Department Chairmen / Division Chiefs to establish ways to monitor, measure, communicate and improve patient satisfaction, physician satisfaction / participation and promote positive, collaborative working relationships. Serves as a liaison between Administration, departments and sub specialties.

  • Serves as faculty for, and participates in the educational and research activities for residents, nurses and other health team members which impact the care of patients within the service. Supports all ongoing research studies within the service.

  • Participates on hospital / health system / community committees, offering expertise in area of accountability. Maintains global knowledge of Professional Standards Committee activities and is available to address issues as they relate to service.

Qualifications

  • Required Doctorate

  • 2 years of relevant experience administration in medical administration, with knowledge of medical business practices. Required

  • Active member of the medical staff of Spectrum Health. Required

  • LIC-Physician (MD) - STATE_MI State of Michigan required Or

  • LIC-Osteopathic Physician (DO) - STATE_MI State of Michigan required
