Make a Difference

The Hospice RN Case Manager is responsible for case management of a group of hospice care clients, providing direct nursing care, and coordinating and supervising other disciplines involved to promote quality services to hospice patients. The Hospice RN is responsible for providing skilled care to their assigned patient according to the plan of care established by the patient’s physician. The Hospice RN practices within the scope of nursing as defined by the Nurse Practice Act.

The Hospice RN Case Manager works closely with hospice staff to meet the needs of hospice patients and their families in their home and in long-term care facilities in central Indiana. This position requires driving to different patient homes and facilities to care for patients in the Indianapolis area.

Exceptional Skills and Qualifications

  • Compassionate as a professional caregiver.

  • Excellent organizational skills.

  • Strong critical thinking and communication skills.

  • Demonstrates ability to respectfully address interpersonal conflicts.

  • Demonstrates customer service skills to provide exceptional patient, and family experience.

  • Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience.

  • Licensed as a Registered Nurse (RN) by the Indiana Professional Licensing Agency required.

  • Current Indiana Driver's License required.

Additional Information:

  • Must be able to attend full week of training first week.
