Join Community

Community Health Network was created by our neighbors, for our neighbors. Over 60 years later, “community” is still the heart of our organization. It means providing our neighbors with the best care possible, backed by state-of-the-art technology. It means getting involved in the communities we serve through volunteer opportunities and benefits initiatives. It means ensuring our dedicated caregivers can learn and grow to stay at the top of their fields and to better serve our patients. And above all, it means exceptional care, simply delivered — and we couldn’t do it without you.

Make a Difference

The Director of Nursing for Hospice coordinates the provision of care through an interdisciplinary team for patients served by our Medicare certified program. This engaging leader is focused on elevating the nursing team as hospice specialists through implementing end-of-life best practices and nursing standards for exceptional care.

The Director will promote Community Health Network’s PRIIDE values to ensure excellent patient care and experience, interdisciplinary dedication to strategic mission outcomes, quality metrics improvement, and empower the team to achieve the program’s goals.

This results-driven candidate oversees compliance with federal and state, regulations and laws, accreditation standards and agency policy and procedures. Accountable for safety and management of risk, this role works closely with education, quality and the Executive Director around any compliance or liability issues.
