Community Physician Network is seeking a Family Medicine physician to join our outpatient practice in Elwood, IN.

• FM Practice plus Inpatient

• Shared call schedule

• Patient Centered Medical Home practice model

• Electronic Medical Record (Epic)

• Ability to practice in a smaller community while having the support of a large network

• Physician-led organization that promotes and supports physician leadership

• Multi-disciplinary physician group of 1300+ providers

• Fully-integrated EMR system (EPIC)

Licensure & Certifications

• Board certified/board eligible in Family Medicine.

• Licensed in Indiana or eligible for medical licensure in Indiana
