McLaren Health Care Social Work Care Manager Outpatient Master of Social Work - McLaren Careers in Detroit, Michigan
Provides direct social work intervention, cooperative interdisciplinary coordination, to/for patients and families in their place of residence and other settings.
KEY RESPONSIBILITIES INCLUDE:
Provides professional social services as medically prescribed to patients and families receiving nursing or other services from McLaren Home Care & Hospice by:
Responding to referrals for social services from McLaren Home Care & Hospice staff and providing services as appropriate. When providing care to patients of the Medicare certified service line, the requirement of CMS is that the Social Worker BSW will be supervised by a MSW.
Will supervise home health BSW by conferencing and cosigning visit notes.
Assessing psychosocial needs of patients and/or families referred for services by reviewing the medical record, interviewing the patient and/or family and consulting with members of the interdisciplinary McLaren Home Care & Hospice team and community providers.
QUALIFICATIONS REQUIRED
Master's of Social Work degree from an accredited program
Valid Social Work licensure in the State of Michigan and proof of insurability.
QUALIFICATIONS PREFERRED
- Two years of experience in hospital, hospice, home care, or geriatric social work services preferred.
