Pickaway County Community Action Organization,Inc. Recovery Support Liaison in Circleville, Ohio

Job Summary:   

The Recovery Support Liaison will be part of a supportive community dedicated to wellness and advocacy.

 

As a Recovery Support Liaison, you'll play a vital role in providing outreach services, coordinating wellness initiatives, and offering referrals to treatment programs. Your work will empower individuals with substance use disorders to make informed choices about their recovery journey. You'll serve as a beacon of support and education for both clients and the wider community, while also fostering sober interactions through coordinating wellness activities aimed at building a positive, supportive environment.

 

Essential Functions:

  • Determines individuals needs by completing intake interviews.
  • Establishes case management plans by coordinating services such as connection to income and benefits, housing stability, employment counseling, recovery  
  • Serves as an advocate for individuals in outpatient and inpatient settings.
  • Assists program participants to become involved within the community.
  • Establishes and maintains relationships with identified community partners.
  • Connects individuals to appropriate treatment options available. 
  • Coordinates regular Wellness activities on individual and group basis. 
  • Coordinates transportation to treatment activities. 
  • Monitors cases by verifying individual's progress toward their goals.
  • Provides additional training and resources as necessary to achieve and maintain goals.
  • Maintains individual's confidential files including case notes; logging events and progress toward established goals.
  • Updates job knowledge by participating in educational opportunities; reading applicable publications; maintaining personal networks; participating in regional meetings.
  • Tracks customer data, and accurately enters the data in the appropriate computer software or program.
  • Reports program outcomes to the director and the funding sources.
  • Assists with grant application, administration and budget. 
  • Plans and coordinates educational and awareness events for community. 

Required Qualifications:

  • High-school diploma or equivalency
  • At least two years of experience in social services, counseling, or related field.
  • Bachelor's or Associate's Degree in Human Services, Social Work, or related field preferred
  • Demonstrated knowledge of community services programs and community resources.
  • Valid Driver's License and the ability to be insured under agency policy
  • Must be able to successfully complete a drug screen
  • Must be able to successfully complete a background check
