Job Title: Nurse Case Manager I

Job Description

-The Nurse Case Manager I is responsible for assisting in the development, planning, coordination, and administration of utilization review and discharge planning activities.

-This includes daily medical records review to assess the necessity of admission, continued hospital stay, and ancillary services.

-The role involves utilizing a collaborative approach to assess, plan, implement, monitor, and evaluate the options and services needed to meet an individual's health needs as prescribed by their provider.

-This position aims to ensure optimal patient outcomes focusing on quality, service, customer satisfaction, and cost-effectiveness, while coordinating a plan of care to promote efficient patient movement through the hospital system.

-The Nurse Case Manager I works closely with a multidisciplinary team to manage resources, support patients and their families, and serves as a clinical expert in discharge planning and case management.

Job Type

This is a Contract-to-Hire position with a duration of 6 Month(s).

Work Site

This is a fully on-site position in Charleston, South Carolina.

Work Environment

-The position primarily involves working with adult patients across various service teams, including surgery, trauma, GI, and general medicine, among others.

-Each Case Manager is assigned to 2-3 service teams, with efforts made to pair teams effectively based on compatibility and specialization.

-The role includes a combination of office and floor duties within a shared office space.

-Preferences for specialty areas are discussed during the interview process.
