Clinical Team Nurse Manager

JOB SUMMARY

As part of the clinical team, the Clinical Team Nurse Manager provides oversight and supervision to the nursing team members. The position is responsible for ensuring the nursing team members are assisting care coordinators with providing health promotion activities, referrals, and assessments. The Nurse Manager ensures the compliance with policies/procedures, documentation, and reporting is met by way of providing administrative and clinical supervision to the nursing team members. As a leader in the agency/clinical department, the Nurse Manager serves as the subject matter expert for physical health and medical information.

ESSENTIAL FUNCTIONS

  • Responsible for overall day to day supervision and leadership of a team of registered and licensed professional nurses who work with care coordinators to provide health home/care coordination services.

  • Serve as a member of various workgroups/committees dedicated to quality/process improvement.

  • Collaborate with Care Coordination Leadership team to design/implement systemic clinical interventions

  • Monitor documentation of clinical team members

  • Conduct routine one on one supervision with nursing team members

  • Conduct routine team meetings with nursing team members

  • Monitor utilization patterns regarding referrals and network providers

  • Design and deliver clinical in-services based on trending and data analysis

  • Responsible for completion and effectiveness of clinical recommendations made by nursing team members

  • Construct and present reports pertaining to clinical outcomes, quality improvement initiatives, interventions

  • Develop and deliver webinars/in-services for the people/families we serve as needed.

  • Operate in a trauma-informed manner

  • Utilize the nursing process to assist nursing team members with interventions/problem solving.

  • Assist with crisis interventions, risk management, crisis planning

EDUCATION & EXPERIENCE

  • RN or BSN with 4+ years serving at risk population required

  • 2 years supervisory experience (required)

  • Experience in Health Home/Care Coordination setting (preferred)
