LaSante Health Center Evening/ Weekend LPN in Brooklyn, New York

FQHC is seeking a qualified candidate for a part-time LPN position. The LPN is responsible for providing support to providers in addressing the health care needs of patients, through the the treatment of responses to actual or potential health problems. The LPN acts as a key member of the care team through the delivery of safe, quality patient care and services.

Job Purpose: As a Federally Qualified Health Center (FQHC) our goal is to be patient-focused and provide integrated services. They should be passionate about serving under-served patients, with an interest in contributing to our continued growth and outreach efforts. The candidate must demonstrate excellent customer service in working with patients when obtaining medical history and vital signs, explaining and assisting in procedures, following up on laboratory results, and referral calls. The LPN must also demonstrate efficiency and knowledge when assisting providers during examinations and procedures.

Job Responsibilities: Room patients, assist physician/provider in setting up for procedures and/or exams and gathering data. Assist and support physician/provider through: Organizing and maintaining an efficient daily operation. Revising daily schedule, as needed, and updating other team members as is appropriate. Obtain reports and other necessary documents for the provider to provide patient care in a safe, caring environment. Administer medications and immunizations in correct dosages as ordered, following the Six Steps of Medication Administration protocol. Observe patient, for reactions, chart appropriately as required. Distribute appropriate educational materials. Collects and documents patient history, vital signs, and other relevant information and reports any abnormalities promptly. Assists physicians with examination, diagnostic procedures and treatment. Performs diagnostic procedures such as EKG, pulse oximetry reading, growth parameters, hearing/vision tests and others as required and within the scope of practice of a licensed practical nurse. Administers medications within the scope of practice of a nurse. Documents appropriately and accurately within the medical records. Assists in organizing patient flow to ensure quality patient care and effective time management. Reinforces the patient and/or family education regarding health promotion and maintenance as well as prevention of illness and injuries. Informs the registered professional nurse of any issues or problems that hinders the delivery of quality care. Disinfects and sterilizes instruments, equipment and reusable supplies. Follow center policies and procedures for infection control and in the handling and disposing of infectious/hazardous waste materials. Maintain inventory of supplies in exam rooms and other areas as designated by the Nurse Manager; i.e. immunizations.

Qualifications and Skills: Minimum two (2) year related healthcare experience Preferred. Current LPN license in the state of New York, Up to date first aid and CPR certification a plus, Possess excellent verbal, written, and interpersonal communication skills, Detail-oriented and a team playeR, Must exhibit passion for outstanding results and compassion for those we work with and serve, Ability to effectively interact with physicians, patients and other staff members Happy, welcoming, and can-do attitude with patients and co-workers, Flexible/adaptable nature to work in a continuously evolving environment Experience with EHR system (eClinicalWorks preferred) Computer Proficiency (including Microsoft Suite) and superior keyboarding skills (approximately 55 wpm) Bilingual is strongly preferred (Haitian Creole, French or Spanish is preferred) Benefits: Competitive Pay Medical Insurance Paid Time Off Flexible Spending Account
