RiseBoro Homecare Inc. Community Health Worker, Marcy Houses in BROOKLYN, New York

ESSENTIAL DUTIES AND RESPONSIBILITIES

RiseBoro Community Partnership’s Wellness Rising program is an ambitious initiative partially funded by DOHMH to promote Health and Wellness education and address health disparities in Brooklyn. The Community Health Worker is responsible for providing education and guidance to the community members within the NYCHA Marcy Houses. Specific duties and responsibilities include:

  • Assist in the preparation of presentations on a variety of h ealth-related subjects

  • Facilitate in-person to individuals within Brooklyn community spaces i.e., libraries, community centers, Family Heath Centers, health fairs, etc.

  • Ensure all program participants complete surveys before scheduled events

  • Remain updated and informed on all health-related material that will be used during the presentation

  • Refer all individuals who request additional health assistance to additional RiseBoro Community Health Worker

  • Update all materials with new information provided by the CDC or NYDOHMH concerning health alerts

  • Maintain accurate daily logs, case notes, and files for all participants

  • Collect and enter client data into the referral database

  • Attend scheduled case review meetings, staff meetings, and assigned/required training

  • Other relevant duties as assigned

    COMPETENCIES

  • Highly Proficient in English/Spanish; ability to read, and write in English and Spanish preferred

  • Experience in community engagement, campaign work, or other forms of direct outreach preferred

  • Experience working in underserved communities with a genuine interest in improving community health preferred

  • Excellent organizational skills and attention to detail

  • Strong communication skills with the ability to speak comfortably and courteously to a diverse population, while maintaining a professional and warm demeanor

  • Excellent interpersonal skills and customer service oriented; ability to maintain positive relationships

  • Possess an entrepreneurial spirit and a positive attitude

  • Ability to ability to accept and followdirections; verbal and written instructions

  • Ability to lift or carry up to 25lbs

  • Valid NYS license preferred

  • Ability to travel to multiple locations in the community with necessary work supplies and materials

  • Familiarity with Bedford Stuyvesant, Brooklyn, NY area preferred

  • Ability to be adaptable and flexible with assigned work schedule; evenings and weekends as required

  • Ability to exercise good judgment and maintain confidentiality, particularly with client information

  • Proficient in Microsoft Office suite, intermediate knowledge in Excel and Word

  • Commitment to RiseBoro’s mission, vision, and values

  • Ability to understand the concepts of institutional and structural racism and bias and their impact on underserved and underrepresented communities

  • Demonstrated commitment to supporting communities that have experienced systemic oppression and bias (i.e., people of color, LGBTQ+ people, immigrants, justice-involved persons, etc.)

    EDUCATION AND EXPERIENCE

  • High School Diploma or equivalent (HSE/GED) required

  • Community Health Worker Certification preferred, Health and Wellness experience will be considered.
