Outreach Development Corporation Full- time Case Manager in Brentwood, New York

Outreach is seeking a qualified candidate for the position of FT Case Manager at our Women's Residence program located in Brentwood, Long Island. The FT Case Manager is responsible for the delivery of care coordination services to an adult population with history of substance abuse, some of whom have specialized needs (chronic medical conditions, veterans, mental health). The schedule for this position is: Sundays 9a-5p, Mondays 4p-12a, Tuesdays 10a-6p, Wednesdays 2p-10p, and Thursdays 10a-6p.

What You Will Do:

· Provide clinical case-management to clients who reside in the Community Residence.

· Counseling and support to enhance clients’ capacity to benefit from treatment regimen.

· Crisis intervention to assist clients in dealing with sanctions, barriers to employment and self-sufficiency.

· Engage in community outreach to locate appropriate clients & assist through enrollment process.

· Assessment and support services to facilitate access to specialized services e.g. Mental, Health, Social Security Disability, Veteran’s Resources, etc.

· Assist in facilitating access to job training, job search activities, placement and stabilization.

· Advocacy and report of client activity to community based organizations and clients’ referral / mandating sources.

· Adherence to agency protocols for maintenance and security of agency equipment.

· Participate in weekly case conference.

· Complete client-centered treatment plans.

· Meet with each assigned client weekly to identify progress towards case management goals.

Who You Are:

· Experience working in a residential treatment setting

· CASAC-T, CASAC, LMFT, LMHC, LMSW, MFT, MHC, MSW preferred, Limited Permit

· 2-3 years of case management experience

· 2-3 years of experience with substance abuse disorders

· 1-2 years of experience working with clients with co-occurring and mental health problems

· 1-2 years of experience working in in an OASAS funded facility a plus

· Commitment to providing quality services and outcome driven performance measures.

· Action-oriented, adaptable, and innovative approach to program planning.

· Clean and valid NYS driver’s license required.
