Hillside Family of Agencies Care Coordinator II- Bath NY in Bath, New York
We have an incredible opportunity for a Care Coordinator II at our Bath, NY location.
This is an exciting opportunity to work as part of a team to start up a meaningful new program (Youth Act) for youth and families where the youth have serious mental health issues. Individual must have experience in teaching skills to youth. Experience helping youth attain educational and vocational goals is a plus, but not required. This position will be responsible for supporting the youth and family in acquiring the necessary skills and abilities to manage their health, improve family relationships and develop opportunities for pro-social activities and interactions.
Responsibilities:
Work as a team member in assessing a youth/family's needs
Develop, assess and modify the individual service plan in collaboration with the youth, family and multi-disciplinary treatment team
Be actively involved in the implementation of a strength-based service plan, including site or home visits and assessments, identify and coordinate collateral resources, recognizing gaps and needs, assess results and communicate with the multi-disciplinary treatment team
Requirements:
Bachelor's Degree in Social Work, Psychology, Sociology or a related Humans Services area of study; Masters is preferred.
4 plus years related experience is required with 5 plus years being preferred.
Must have thorough knowledge of the services provided, well developed advocacy skills, and an understanding of payment processes for services
Must have a valid New York State Driver's License and meet agency driving and insurance standards.
$20.18 Minimum Rate per hour, $30.28 Maximum Rate per hour, based on experience.
