The Associate OR Masters Licensed Social Worker/Counselor/Mental Health Clinician provides counseling to patient’s in a variety of settings including medical facilities, hospitals, clinics, learning centers and other organizations that are in need of assistance.

Essential Duties and Responsibilities:

• Provides necessary counseling as required for the patient/client

• Assesses the patient/clients’ psychosocial needs, and evaluate the progress with the care team and/or supervisor

• Collaborates with all disciplines to plan and evaluate goals for each patient/client

• Focuses on assisting patients/clients through the discharge process of both in and out-patient treatment facilities as applicable

• Utilizes a variety of therapy techniques and treatment plans

• Facilitates patient progress and referrals with other agencies

• Adheres to all Amergis and Facility policies and procedures

• Observes and reports patient/client progress per facility protocol

• Performs other duties as assigned/necessary

  Minimum Requirements:

• Licensing/Certification according to state/facility/contract requirements

• The Clinician may possess an Associate’s Degree, Bachelor’s Degree or Master’s Degree in Psychology, Sociology, Social Work, Counseling, Nursing or other Human Development Major

• Complies with all relevant professional standards of practice

• Participation and completion of Amergis' Competency program when applicable

• Current CPR if applicable

• TB questionnaire, PPD or chest x-ray if applicable

• Current Health certificate (per contract or state regulation)

• Must meet all federal, state and local requirements

• Successful completion of new hire training as applicable to job site

• Understand patient confidentiality and HIPAA requirements

• Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

• Computer proficiency required

• Must be at least 18 years of age
