ILLUMINATION FOUNDATION Lead Case Manager in Anaheim, California

Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description

The Lead Case Manager is responsible for assessing the strengths and needs of homeless clients, developing care plans for clients within their service area, and overseeing client care within their assigned program. The Lead Case Manager will work closely with the Program Manager to ensure contract compliance and outcomes. 

The pay range for this position is $26.00 - $28.00 per hour, depending on experience. 

 

Responsibilities

Client Care Oversight

  • Develop care plans for clients within assigned programs.

  • Oversee the care plans in partnership with the Program Manager by conducting monthly meetings with care teams

  • Oversee client assessments to evaluate client’s need, develop treatment plan for client, monitor progress, provide assistance and referrals, set-up and follow-up with weekly goals for case managers to work on with clients.

  • Provide crisis intervention for all clients within program are

    Direct Client Care

  • Meet with clients as often as needed to carry out the goals of the care plan

  • Assist clients in connecting to any basic needs resources

    Supervision

  • Supervise assigned cms weekly to discuss client care and assist in strategic problem solving to overcome barriers

  • Conduct monthly care team meetings with all relevant members of the client’s care team

  • Provide direct case management to participants as needed; supervise all case management activities carried out by employees, volunteers, and interns to ensure participants receive excellent service.

    Administrative/ Record Keeping

  • Track client and program data using Google docs and HMIS

  • Create, maintain, and organize client case files with necessary documentation

  • Complete Client Outcome Matrix when due and input into HMIS

  • Complete weekly and monthly reporting

  • Use outcome measurements

  • Maintain updated and accurate case notes

  • Complete BPS forms, weekly goals, and exit forms

  • Create, fax, email and send documents on client’s behalf

  • Attend weekly individual supervision session with immediate supervisor to discuss program organization, client’s progress, new strategies to meet client’s level of need and improve treatment plan

    Collaborations

  • Participate in Case Management Team Meetings and staff meetings

  • Network with community providers

    Preferred Experience/ Minimum Qualifications

    Required:

  • 2 years of experience and/or Bachelor's degree in Social Services, Healthcare or related field experience.

  • Homeless services, case management, mental health, permanent supportive housing experience

  • Valid CA drivers license

  • At least 1 years of experience working with at risk/homeless individuals (preferred)

  • Must be familiar with VI-SPDAT.

    Preferred:

  • Bilingual in English and Spanish.

  • Experience in the area of non-profit housing and/or housing for people with disabilities and chronic health conditions

  • Master's degree preferred

  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Teams)
