Working cooperatively with other agencies such as Doctors/Nurses, Home Health (PT/OT) rehabilitation, additionally; Hospice palliative and comfort care (end of life). Supporting both our clients and their families with Relationship Resources from our Resource Room Our Resource Room provides information to not only assist our clients, but our caregivers as well!

Job Description:** Our job is to enable our clients to stay in their homes by assisting with Activities of Daily Living (ADLs) which include: personal care (bathing, dressing, electric shaving, transferring, walking support), Meal preparation, Medication reminders, Light Housekeeping.

Additionally we provide transportation for shopping, and doctor appointments (use of own vehicle provides you with $.75/mile reimbursement) We monitor and record factual observation of our clients' conditions or changes in their baseline mental/physical behaviors Just as importantly, we offer our skills and encouragement of coaching the clients' families basic care assistance for their loved ones (re-positioning) and with supportive Relationship Resources throughout our networks.

Duties: * Monitors patient condition by observing physical and mental condition, intake and output, and exercise.

  • Supports patients by providing housekeeping and laundry services; shopping for food and other household requirements; preparing and serving meals and snacks; running errands.

  • Assists patients by providing personal services, such as, bathing, dressing, and grooming.

  • Helps patients care for themselves by assisting with use of cane or walker, special utensils to eat, special techniques and equipment for personal hygiene.

  • Helps family members care for the patient by assisting with or supporting appropriate ways to lift, turn, and re-position the patient; advising on nutrition, cleanliness, and housekeeping.

  • Records patient information by making entries in the patient journal; notifying supervisor of changing or unusual conditions.

  • Updates job knowledge by participating in educational opportunities.

  • Protects the home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements.

  • Enhances service reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
