Transitional Care Team Nurse (LPN)

Maxim Healthcare Services,Inc., an ACHC Accredited Homecare Agency, is seeking experienced and passionateLPN candidates to fill select-teams of Transitional Nurses. TransitionalNursing Teams support and care for medically fragile children and adults asthey transition from hospital care to home care. As a member of theTransitional Team, you are seen as a nurse that possesses something that isspecial and unique. We believe that you have the appropriate skills requiredwhich must include tracheostomy care, ventilator care, and various othersupport through advanced portable therapeutic devices.

All of our Transitional Teamnurses have been carefully sourced, and not everyone is selected for theteam. Those who are chosen are candidates who we feel possess nursing andcustomer service skills that are more advanced than traditional private dutyhomecare staff. Those of whom are selected are focused on excellent patientcare, quality customer service, and a desire to continue growing within theirfield. The heart of this team will be patient and family focused with thegoal of ensuring a smooth transition home from the hospital. Since there is much responsibility andcommitment while working in this role, we will be offering a higher payrate.

While at Maxim HealthcareServices, you will be working as an LPN to assume responsibility andaccountability for the application of the nursing process and the delivery ofpatient care, under the supervision of a Registered Nurse (RN). As aTransitional Team member, you will consistently perform according to nursingstandards and will be held accountable in managing patient care and assistingothers in the management of patient care.

Expectations andResponsibilities:

  • Consistentcommunication with internal staff

  • Ongoingvigilance with securing PHI, meeting all HIPPA regulations, and security ofpatient and staff privacy

  • Abilityto maintain a schedule that will change frequently

  • Possibilityof lengthy commute times to and from work

  • Fortitudeto work in varying home environments for up to 90 days

  • Mustbe able to work a minimum of 24 hours per week

  • Mustbe able to work a minimum of 4 weekend shifts a month

  • Utilizesthe nursing process to assess, plan, implement and evaluate patient care

  • Performsfocused interview to identify specific patient needs

  • Assesssigns and symptoms indicating physiologic and psychosocial changes in thepatient’s condition

  • Collects,analyzes, and interprets data and information from health care members anddocuments actual and/or potential nursing diagnoses

  • OtherLicensed Practical Nurse (LPN) duties as assigned.


  • Current Licensed Practical Nurse(LPN) License in the state in which the Licensed Practical Nurse (LPN)practices

  • Current PPD or Chest X-Ray

  • Current BLS card

  • Ideal candidate will display exceptional customer service,communication, and professionalism qualities

  • One year prior Licensed PracticalNurse (LPN) experience preferred

  • Must pass office competency/skills validation on thefollowing items:

  • Trach

  • Vent

  • G-Tube
