Forest / Warren Human Services Intellectual Disabilities Caseworker 1 or 2 in WARREN, Pennsylvania

Provides intellectual disabilities service coordination services to adults and families to help them attain a more satisfactory social, economic, mental or physical environment.

Qualifications:         

Caseworker 1:    A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; OR two years experience as a Social Services Aide 3 and two years of college-level coursework which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; OR any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a  Social Services Aide 3 or similar position performing paraprofessional case management functions.

Caseworker 2:  Six months as a Caseworker 1; OR successful completion of the  Social Casework Intern Program; OR a bachelor's degree with a social welfare or social work major; OR a bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social services agency; OR any equivalent combination of experience and training.

Duties and responsibilities (include but are not limited to): * *

  • Conducts individual intakes, assess strengths, interest and ability levels by conducting interviews with individuals, family members, and friends.
  • Completes the Adaptive Behavior Scale.
  • Facilitates the identification of specific goals and objectives as identified by the individual and supportive persons through the Individual Supports Plan (ISP) process.
  • Responsible for writing and updating the ISP at least on a quarterly basis.
  • Facilitates the application of positive approaches by empowering individuals to select realistic life choices based on personal interests and preferences.
  • Promotes person and family-driven services.
  • Makes referrals to specialized and generic community services to assure individual needs are met.
  • Completes a Prioritization of Urgency of Need for Services (PUNS) on an annual basis, or as needs change.
  • Coordinates services and supports to enable persons to attain their highest level of self-sufficiency while achieving social integration in the community, while maintaining security, safety and permanency in the community.
  • Provides case management services to assure an adequate and stable income, and meet citizenship responsibilities such as tax returns. May develop personal budgets and monitors expenditures.
  • Maintains a thorough understanding of available services, providers providing these services and current service definitions.
  • Understands different funding options to include Consolidated Waiver, Person Family Direct Support Waiver, Base funding, and Family Support Services.
  • Maintains documentation of all contacts.
  • Completes the required documentation for Targeted Service Management Medical Assistance billing.
  • Attends required trainings and meetings
  • Independently responsible for conducting on-site visits approximately 60% of time performing this function.
  • Participates and coordinates, when necessary, in interdisciplinary team meetings on appropriate clients.
  • Promotes and explains federally-funded waiver program at Intake and at least annually to all BSU individuals not already in waiver.

Crosses program lines when the need arises. - Acts as a liaison between the individual/family, service providers, community services, school district. - Work toward a trauma informed, coordinated, and collaborative system of services and support driven by the youth, adults, and families it serves.

 

Required Knowledges, Skills, and Abilities:

  • Knowledge of community resources.
  • Knowledge of social, economic and health problems and corresponding resources.
  • Knowledge of federal, state and county rules, regulations, policies and procedures.
  • Knowledge of Agency practices and procedures.
  • Knowledge of computer software, such as: Microsoft Word, Excel, Outlook, etc.
  • Ability to establish and maintain positive working relationships with clients, families, agency staff and other community agencies.
  • Ability to implement developed training programs.
  • Ability to follow oral and written instructions.
  • Ability to communicate effectively both orally and in writing.
  • Ability to provide structured and unstructured life skills instructions and guidance to consumers as needed.
  • Ability to walk on uneven ground, uphill grades and climb stairs.
  • Ability to sit and/or drive for long periods.
  • Ability to bend and lift up to 40lbs.  
  • Ability to reach above shoulder level and below waist level.
  • Ability to operate a personal computer, cell phone and digital camera.
