Position Summary/Description:

This position will support the [ CATCH -Johnston] program. The Patient Access and Collaborative Care Coordinator is responsible for supporting care delivery as part of the interdisciplinary care team through supporting care delivery at the virtual care community access points, patient resource coordination, and care planning. Applicants may also be asked to complete tasks assigned by the Program Director and the Medical Director to support [ CATCH -Johnston]. Such tasks include, but are not limited to, connecting with stakeholders, community education, direct patient outreach, quality metric reviews, and preventative gap closure. Team members will utilize UNC’s electronic medical record.

Education and Experience:

Associate’s degree in a related Human Services discipline and two years of experience related to the area of assignment, or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions.
