This Master's prepared behavioral health professional will implement, monitor and document therapeutic interventions, develop overall treatment/action plans, coordinate physician/care team-developed medical treatment plans, and utilize data metrics related to treatment/outcomes. The Clinical Case Manager will facilitate referrals within LVHN and throughout the community in order to address patients' health as well as social needs.Minimum Requirements: Masters degree in a related field AND/OR Masters/Graduate degree in a clinical discipline including but not limited to psychology, counseling or nursing. (Licensed Professional Counselor (LPC).Minimum one (1) to three (3) years work experience as a behavioral health provider/therapist or social worker in a mental health related field. Experience as a behavioral health provider/therapist in a mental health related field. Experience working in an ambulatory medical practice setting or outpatient facility. Counseling, knowledge of medical and community resources, referral sites, and individual and group activities involving patients with substance abuse history and treatment modalities.Must demonstrate leadership skills in working with other health care providers and clinical and non-clinical staff members to attain goals of an integrated program focused primarily on pregnant and postpartum women. Must demonstrate excellent customer service/listening skills especially empathy and involvement in working with an at-risk population. Will support data tracking and perform case coordination with physicians, nurses, other members of the care team and community resources. Must be flexible with assignments and scheduling to accommodate the needs of patients.Preferred Qualifications: Masters of Social Work (MSW) degree from a program accredited by the council on social work education is helpfulFour (4) years direct experience preferred.
