Description:
The Primary Care RN (RN Case Manager) provides care management, coordination, skilled care and services for at-risk frail elder patients with complex medical and psychosocial issues.
Managing a patient or participant case load of 60-65 participants.
Patient assessment, Care planning, Implementation, Coordination and monitoring an ongoing routine evaluation that ensures medically and financially appropriate care.
Serves as liaison to the patient's caregivers, including family members and other staff of the PACE network.
Facilitating interdisciplinary team care coordination, intensive medical management of participants recently discharged from hospital and skilled nursing facilities.
Provides mentoring, education, anticipatory guidance and nursing delegation to community caregivers and other team members in the application and implementation of the nursing care plan tailored to the specific needs of the caregiver's scope of practice.
Work closely with Social Workers, Community Care RNs and Providers, to collect the necessary medical and social information to ensure comprehensive decisions are made which ensure the safety and well being of participants.
Acts as a patient advocate to ensure that needed health services are provided in a timely manner.
Work to effectively assist patients, families, caregivers and other support persons in finding satisfactory solutions or adjustments to medical, social, emotional problems. Takes into account the individual physical, cognitive, emotional, social, ethnic, cultural, religious, gender and age-related needs of the patient.
Identifies, researches and/or refers potential quality of care issues.
Consistently focuses on improving nursing care quality by readjusting care strategies and priorities.
Documents ongoing follow-up and effectiveness of the patient's care/treatment plan.
Skills & Qualifications:
MUST HAVES:
OR RN License
BLS Certification - American Heart Association
Experience with EMRs - Epic experience is preferred
1+ year(s) of Nursing experience in any of the following specialties: geriatric, home health, long-term care, skilled nursing facility, community health, hospice, acute care, med-surg, wound care, or other inpatient RN role
Hands-on experience doing comprehensive nursing assessments
Works well with teams/team player
Strong collaboration skills/experience
Strong communication skills both written and orally
Knowledge of nursing process/clinical problem-solving process & disease management principles
