THE POSITION

Child & Adolescent Caseworker Outreach Program Case Management is designed to ensure access to community agencies, services and people whose functions are to provide the support, training and assistance required for a stable, safe and healthy community life.

DESCRIPTION OF WORK

The Child and Adolescent Outreach Caseworker responsibilities include monitoring and assessing needs, working closely with all behavioral health and ancillary service providers, providing consultation in accordance with applicable confidentiality regulations, providing prevention and intervention activities in schools and community. You will also participate in community groups and coalitions, disseminating educational materials, serving as the Mental Health Liaison to the Student Assistance Program Teams in assigned schools, maintaining documentation and follow-up as needed.

The Child & Adolescent Caseworker Outreach Program Case Manager will assure appropriate referrals are made to assist the child or adolescent in accessing more formal service including, but not limited to Mental Health Blended Case Management and/or Drug & Alcohol Case Management, when necessary. Other duties will be performed, as related, to complete program objectives.

Work Schedule and Additional Information:

  • Full-time employment.

  • Work hours are 8:00 am to 4:00 pm, Monday - Friday, with a 30-minute lunch.

  • Hours may vary due to travel, after hours appointments, etc.

  • Telework: You may have the opportunity to work from home (telework) full-time. In order to telework, you must have a securely configured high-speed internet connection. If you are unable to telework, you will have the option to report to the headquarters office in Towanda, Pennsylvania.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences;or

  • A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences;or

  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.

  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

    Other Requirements:
