Gentiva Care Coordinator Non Clinical - WFH in Mooresville, North Carolina

Overview

We are looking for a Care Coordinator Non Clinical to join our team. This position will directly report to the Director Contact Center and is responsible for taking non clinical inbound calls and escalation to a nurse, as well as to support referral coordination for patients who need Gentiva Services.

Schedule is 8am-8pm Saturday and Sunday and 8pm-8am Saturday/Sunday

  • Able to navigate healthcare options; care services post-acute offerings, Medicare coverage, billing issues, as well as accessing healthcare resources.

  • Utilize a variety of tools and methods to quickly provide patient options and education including but not limited to sites of service, specialty offerings, post-acute care, and other related questions.

  • Assists in admissions process by working closely with the Gentiva branch staff to follow up on patient admission status.

The shift is : 8am-8pm Saturday and Sunday and/or 8pm-8am Saturday/Sunday.

About You

  • Associate degree preferred

  • Minimum of 1 year working in a healthcare related field or healthcare call center

  • INTERNET REQUIREMENT: High-speed Internet connection (minimum 10 Mb/s download speed and minimum 2 Mb/s upload speed, recommended 5 Mb/s upload). Satellite based internet services are not acceptable.

  • Excellent analytical and problem-solving skills.

  • Ability to learn and master information related to locations and services of client.

  • Outstanding computer skills.

  • Good time management skills

  • Good working knowledge of home health, hospice and palliative care services

  • Ability to communicate effectively with empathy over the phone and while interacting with others; excellent interpersonal skills.
