Fairview Health Services Team Coordinator in Maple Grove, Minnesota
Fairview Health Services is looking for a Team Coordinator based out of the Fairview Clinics- Bass Lake in Maple Grove, Minnesota. This position will also support the Brooklyn Park and Andover locations. This is a full time benefit eligible position.

As a member of the care team the team coordinator performs quality patient- and family-centered care principles through performing a variety of high level functions within a care team to support the needs of the healthcare team and patients/families. The team coordinator manages workflow by coordinating and maintaining provider schedules, uses a variety of communication tools and systems to handle medical information requests and updates, communicates normal test results either through the direction of the RN or provider or by following established protocols, and works collaboratively with the assigned unit while handling multiple tasks, interruptions and changes. As a member of the care team, the team coordinator will maintain complete, timely and accurate documentation for each patient and continually look for opportunities to improve processes and workflow
Manage patient and provider schedules for assigned area
Complete patient registration/check-in process.
Use effective communication tools and skills
Manage multiple technology applications
Multi-task and prioritizes appropriately
As a member of the care team, continuously improve work processes, environment and the overall experience to serve customers and meet business goals
- Successful competition of formal medical training program such as MA, HUC, NA or Medical Secretary, or minimum of 1 year comparable experience in a healthcare setting
- 2 or more years’ experience in a medical setting
