Schedule Weekly Hours:40 Full Time Position - 1.0 FTE (80 Hours every two weeks) Shift: Days, between the hours of 7:00am - 6:00pm, Monday through Friday Frequent travel within the service area is a part of this position. Job Description: The Community Paramedic (Paramedic - Mobile Health) functions as a collaborative team member under the supervision of the Medical Director and Director. The Community Paramedic provides acute episodic and transitional care to assist the patient in attaining optimal functioning. The Community Paramedic independently assesses, interprets findings, reports and initiates treatment under the direction of the medical provider. The Community Paramedic provides treatment and care for patients of all ages in accordance with established policies, protocols, and standards to provide quality care. Major Responsibilities: 1. Collaborates with medical providers in inpatient (acute care) and outpatient departments to identify patients who may benefit from acute episodic or transitional care (example- high risk for readmission). 2. Must have ability to work independently with frequent interruptions, under stress, and with tight deadlines. Ability to comprehend and analyze details. 3. Administers acute episodic or transitional care to patients in the home setting. 4.Assesses patient and family knowledge of and compliance to disease management and plan of care. Addresses and supports cultural practices as long as such practices do not harm others or interfere with the planned course of medical therapy. 5. Educates patients and families to understand the illness and treatment options, as well as consequences of various treatments or treatment refused. Uses knowledge of complex physiology to monitor, detect and anticipate early and subtle health status changes. 6. Partners with patient to identify barriers and action plans to optimize health and adherence to plans. 7. Conducts home safety and social risk (SDOH) assessment. Using motivational interviewing strategies, identifies risks and intervenes, and makes referral to community resources. 8. Conducts physical assessment, medication, and appointment adherence. Identifies risks and intervenes as necessary to enhance patient outcome. 9.Initiates protocols and implements treatments as needed. 10.Collects, interprets, and reports Point of Care lab data. Also performs routine quality assurance testing. 11.Demonstrates effective communications skills within the healthcare team.
