Brooks Rehabilitation Registered Nurse , Acute Care at Brooks Home Care in Jacksonville, Florida

Provide advanced nursing care for patients within an acute care at home model. The targeted patient population includes conditions such as congestive heart failure, chronic obstructive pulmonary disease, diabetes, hypertension, hematology/oncology care and support, and complex wounds. May care for pre and post operative patients or perform advanced wound management and therapeutic treatment and procedures including phlebotomy, ostomy care and management, intravenous, enteral, total parental nutrition administration. The Acute Care RN provides clinical care as ordered by physicians and follows documentation requirements and time-lines within the Advanced Care at Home model of care.

The Acute Care RN will be available during the assigned shift to meet staffing requests for acute patients in a home setting. The Acute Care RN works collaboratively with the ACH Command Center team, the Acute Care Charge Nurse, the Clinical Team Manager and Director of Nursing to ensure the delivery of quality and efficient care. This position must develop a strong working knowledge of the Acute Care at Home model of care and specific needs of the patient population. The Acute Care RN will have a strong knowledge of Medicare home health regulations, as well as reimbursement methodologies within the Advanced Care at Home model of care.

  • Utilize the nursing process to provide nursing services to the patient in the home setting under the direction of the managing physician team

  • Assess and treat acute or chronic conditions that could result in rapid physiological deterioration or life-threatening instability.

  • Assess and treat acute illnesses, infections, or injuries.

  • Assess and analyze patient data to determine patient needs and/or treatment goals in accordance with the physician ordered plan of care

  • Assess urgent and emergent health conditions using both physiologically and technologically derived data.

  • Document data related to patients' care including assessment results, interventions, medications, patient responses, or treatment changes in the patient’s medical record.

  • Assess the impact of illnesses or injuries on patients' health, function, growth, development, nutrition, sleep, rest, quality of life, or family, social and educational relationships.

  • Assess the needs of patients' family members or caregivers.

  • Educate patient and family as per the plan of care to improve ocnditisn condition and increase wellness

  • Monitor patient conditions during treatments, procedures, or activities.

  • Set up, operate, or monitor invasive equipment and devices such as colostomy or tracheotomy equipment, surgical drains, catheters, gastrointestinal tubes, and central lines.

  • Administer fluids and medications as ordered, to include into include intravenous and other routes

  • Observe patient reactions to medic or medication changes as needed.

  • Manage patients' pain relief and sedation by providing pharmacologic and non-pharmacologic interventions, monitoring patients' responses, and changing care plans accordingly.

  • Obtain specimens or samples for laboratory work

  • Treat wounds or superficial lacerations in accordance with the physician ordered plan of care

  • Prepare medical supplies or equipment for use.

  • Perform emergency medical procedures, such as basic cardiac life support (BLS), and other condition stabilizing interventions.

  • Communicate detailed medical information to patients or family members.

  • Collaborate with members of multidisciplinary health care teams to plan, manage, or assess patient treatments.

  • Collaborate with the team to assist patients in organizing their health care system activities

  • Participate in patients' care meetings and conferences as needed

  • Perform administrative duties that facilitate admission, transfer, or discharge of patients

  • Participate in the development of practice protocols.

  • Provide formal and informal education to other staff members.

  • Maintain medical or professional knowledge.

  • Crossed trained to support skilled home health needs including OASIS assessments, subsequent visit needs, and same day delivery of care services as needed

  • Current licensed Registered Nurse in the state of Florida, Bachelor’s degree preferred

  • 2 year of acute hospital nursing experience required, at least one year of home health nursing experience prefer.

  • Excellent verbal and written communication skills

  • Strong ability to perform nursing tasks such as the administration of intravenous therapy, phlebotomy, wound care within the home care environment

  • Valid Driver licensed

  • Current BLS certificate
